Here is what you will need for check in, which will take place on from 6 to 8 PM at the West Des Moines Sheraton on 1800 50th St in West Des Moines, IA.
State-approved roster
Permission to travel form if team is from out-of-state
Guest player forms
Passes for all players including guests
Medical release forms for each player (including guest players)
