HOTELS:  Hotel Reservation must be made through the tournament sight.  Failure to do so may result in your team not be accepted into the tournament.

Permission to travel form if team is from out-of-state
Guest Player Forms (if applicable)
Passes for all players including guests
Medical release forms for each player (including guest players)
